3 Phases of JHUM

LifeScripts is the phase I of our program. This connect-to-college (C2C) resource tool is a six-week workshop designed for foster youth who have graduated from high school and are pursuing their college education.

Admissions requirements for City College: · At least 18 years old · High school graduate · Completed your GED or CHSPE (California High School Proficiency Exam) · High School Enrichment Program is available to high school juniors or seniors with permission (foster parents, high school counselor, and principal). Each week we will work with qualified AFY students who desire to enroll in City College and successfully transition into the first-year college experience.

Six Weeks in Six Days: (Workshops, resources, and tools) surrounding these six areas: Application, Orientation, Assessment, Ed Counseling, Registration, and FAFSA- Financial Aid. This workshop also includes career-oriented Ed planning, college tour, schedule, and certificate of completion.

Jacob’s House Urban Missions is a community-based organization that assists aged-out foster youth in obtaining after-high school education. The three phases in our program are 1. Lifescripts, which connects students to college. 2. AFY College Life is a two-year housing and educational support service; and 3. CPST is a coaching practicum that assists students in a sustainable transition to the community. We are committed to breaking the cycle of poverty for this student population by making a significant difference in the lives of young people exiting foster systems in our city.
